“Dear Transfer Agent, please use the opinion letter from MDWerks, Inc. counsel on file to remove the restrictions from my shares. Please place my shares into DRS format. Included in this email you will find attached my completed seller representation letter. Please return a statement to me showing that my shares have been unrestricted.”
